Special Education Services Tracking Form for use During School Closures



The Arc of the United States has shared a great resource with us that we would like to share with families. This form was created to help parents/caregivers track any services provided to their children during the school closures to make things easier when they return to school. State guidance says that, any missed services and regressions will be addressed at an IEP Team meeting once students return to school so that the IEP Team can determine compensatory services to make up that lost time. Having these forms filled out for each week of missed school will be very helpful.
